HCG is FDA approved to treat a low testosterone or Hypogonadism stemming from hypo-functioning of the pituitary gland. This is a good option for some men who feel taking testosterone replacement therapy is “taking steroids” and they don’t want to be associated with this stigma. HCG therapy is a valid from of testosterone replacement therapy without actually taking testosterone. The dose of HCG necessary to elevate testosterone back to normal levels is 500-1000 units 2-3 times a week.
HCG works by switching on the pituitary gland and thereby stimulating the testicle to produce its own natural testosterone.
Again, the down side is you have to perform self-injection but the injections are not every day and it is essentially painless because it is done with a insulin syringe with a very short needle. As with all injections there is a chance of skin infections. If a patient opts to use this option we will instruct you in the technique of safe self injections.
Another use for HCG is to prevent testicular atrophy. Men undergoing testosterone replacement with testosterone with injectable, topical and pellets forms all have a risk of testicular atrophy. Atrophy occurs because the testicle is no longer producing its own and it shrinks. To prevent this from occurring, these patients are treated simultaneously with a low dose of HCG three times a week.
HCG is a good “first step” to consider before testosterone replacement therapy and is prescribed for the following reasons:
To help treat adult men experiencing hypogonadotropic hypogonadism (hypogonadism secondary to a pituitary deficiency) / Andropause / Sarcopenia.
To help treat adult men who are potential TRT candidates based on symptoms and problems but their blood test results show Total and Free testosterone levels are normal.
HCG therapy better mimics the body's own natural physiologic rhythm of testosterone production.
HCG therapy is easier to maintain normal testosterone levels when administered properly.
More physiologic testosterone levels minimize excess estrogen production (i.e., reduces aromatization) preventing high estrogen related side effects.
Maintains normal size of testicles (in contrast, standard testosterone replacement therapy may shrink the testicles).
Stimulates sperm production (thereby increasing/restoring fertility). In contrast, standard Testosterone Replacement Therapy may reduce sperm production.
